Draft: Cleanup and remove 'append'-hack in favor of SMW hook

This pull request removes the infamous "append"-hack in favour of using a hook provided by Semantic MediaWiki. This will greatly improve performance on all sites. Sites will also no longer break when visible content is inserted in a slot.

This pull request removes the configuration value $wgWSSlotsSlotsToAppend in favour of $wgWSSlotsSemanticSlots.
